The Health Outcomes Survey (HOS) is sent annually to a sample population of your patients to learn more about how your practice supports improving their physical and mental health over time. The 2020 survey will be sent in August, so we would like to point you to resources to support your interactions in 2020 and beyond!
HOS is a Centers for Medicare & Medicaid Services (CMS) requirement and is administered to a random sample of each health plan’s Medicare members. UnitedHealthcare reviews the survey results closely every year, and the insights we learn are applied to our work with providers.
When you support your patients’ overall well-being, it will be reflected in your HOS results, as well as your CMS Star Ratings. HOS results will make up 12 percent of a health plan’s CMS Star Ratings for the 2020 survey year.
